St. Peter's Health Partners has an excellent full-time employment opportunity for an experienced Nurse Practitioner or Physician Assistant to join the Palliative Care Partners, a service line across St. Peter’s Health Partners, located in Albany, New York.
The palliative care team supports patients and their families who are dealing with a serious illness. The practitioner will provide palliative care services in the hospital inpatient setting.
The selected candidate will work closely with an interdisciplinary team including an MD, social worker, and spiritual care support to provider comprehensive care to patients with advancing illness primarily in the hospital setting. The NP or PA will work with consultant and practice staff to manage pain and symptoms, address goals of care and treatment plans, and provide guidance and education to patients and families.
Palliative Care Partners provides a supportive work environment and thorough orientation. The work schedule for this opportunity is Monday – Friday, 40 hours per week, 8 am to 4:30 pm.
The Ideal candidate will be Board Certified and must be able obtain New York State licensure and DEA. Candidates must have two years of NP or PA clinical experience, preferably in palliative care and home health.
