St. Peter’s Health Partners has a full time employment opportunity for a Board Certified Nurse Practitioner with Geriatric Medicine experience or training to join its established program located in Capital District, New York.

The Eddy Senior Care, a member of St. Peter's Health Partner’s, is an established and respected 20 year PACE program (Program of All Inclusive Care for the Elderly) that is seeking a full time Board Certified Nurse Practitioner to join its team for immediate placement.

This is an excellent opportunity to join a unique practice environment which provides the opportunity to work with an interdisciplinary team to support frail seniors who remain living in the community and thus avoid nursing home placement. It is a program driven by relationships – not the number of encounters. The Nurse Practitioner follows the participant whether at home, in the medical office, or in selected nursing homes should placement be needed.

The schedule for this opportunity is full time, Monday – Friday position with shared weekend and weeknight call rotation.

Applicants should be Board Certified in Adult Gerontology or Family Nurse Practitioner trained and have a NYS License and DEA certification. A minimum of 1-2 years of experience working with an elderly population and a minimum of 2-4 years of experience working in a primary care setting is also required.
