Nurse Practitioner - Palliative Medicine Opportunity - 1883 (21206254)

Saint Agnes Medical Center (SAMC) has an excellent employment opportunity for a full time Nurse Practitioner to join the Palliative Services team located in Fresno, California.

The selected Nurse Practitioner will report to the Director of Palliative Services and Medical Director of Palliative Services. The Nurse Practitioner is responsible for providing health care for the palliative patient population as part of the health care team. Oversight of the functions and duties of this role will be provided by the Medical Director for Palliative Services. In accordance with the mission of Saint Agnes Medical Center, the Palliative Nurse Practitioner provides clinical support, education and development, case management and supervisory functions for Saint Agnes Palliative Care Services, primarily in the hospital setting.

The Nurse Practitioner is responsible for the following:

Nursing Process
- Support palliative physician consultations for hospitalized patients needing Acute Palliative or Hospice Palliative Care by obtaining patient history, relevant family background and legal surrogate information; and clarification of patient/surrogate understanding of illness and treatment options.
- Facilitate or co-facilitate family meetings related to current and advance care planning.
- Assess, plan, implement and evaluate care of hospitalized patients of Saint Agnes Hospice during inpatient course.
- Rely on standardized procedures to perform overlapping medical functions, developed in collaboration with the supervising physician.
- Order drugs and devices under physician or surgeon supervision as authorized by standardized procedure or protocols.
- Provide administrative supervision and program oversight for Palliative Services nurses and staff.

Education
- Coordinate/facilitate interdisciplinary education acting as a consultant and mentor in evidence-based palliative care.
- Act as role model and resource related to current standards of practice in palliative care.
- Mentor/educate medical, nursing, and other professional students in principles of palliative care.
- Oversee orientation modules on Acute Palliative Care for newly hired clinicians, medical and nursing students, and residents.
- Provide current patient/family information related to accessing Palliative Care Services.

Continuity Planning
- Ensure completion of rounding instrument daily to provide timely information for the interdisciplinary team related to outcomes of palliative interventions according to patients' report and family observation.
- Assist with discharge preparation and enrollment into Hospice for patients to ensure access and continuity of care.

Collaboration
- Collaborate with attending physicians, hospital staff, consultants, Palliative Physicians/Hospice Medical Directors, and Hospice Interdisciplinary Team regarding patient care, treatment, services.
- Participate in interdisciplinary meetings/rounds so that care involving management of complex, end-stage diseases; pain and symptom management; need for psychosocial/spiritual support; and caregiver support related to patient/family ability to provide care are provided according to established protocols.
- Assist in development of policies, procedures and clinical guidelines.
- Collaborate with hospital and hospice educators in developing competency of inpatient palliative care staff in pain and symptom management.
- Initiate appropriate referrals to chaplain, dietitian, pharmacist, consulting physicians, ancillary therapists and other services as per criteria and/or as authorized by Hospice Interdisciplinary Team.
- Participate as member of Palliative Care Work Group and Palliative Care Consultation Team meetings.

Safe Practice
- Verify prior to implementation, physician/other licensed independent practitioner's orders.
- Administer medications, according to the organization's clinical and safety standards, policies, procedures, and/or guidelines 
- Mentor safe and appropriate use of opiates and other analgesics adhering to guidelines related to physician-directed titration schedules.
- Initiate appropriate emergency procedures as per policy and in accordance with patient/surrogate informed consent.
- Report and/or remove non-functional equipment.
- Adhere to standardized procedures and approved formulary.
- Identify high-risk areas regarding patient/family satisfaction, safety and follow policies related to high risk/low volume/problem prone procedures.
- Comply with the error reduction and reporting processes.
- Inform upwards in a timely manner identified high-risk situations.

Documentation
- Document information related to consults performed individually or in conjunction with Palliative Physicians.
- Ensure documentation of the Interdisciplinary team with rounding and team conference findings and outcomes.
- Document accurately and completely and in a timely manner pertinent assessments, interventions, plans, and outcomes for patients in accordance with documentation standards, policies, procedures, and/or guidelines.
- Submit documentation, reports, and daily activity reports within timeframes as determined by policy/guidelines.
- Ensure validity of database by documenting complete, accurate, and timely information that can be readily entered into the Palliative Care database.

Resource Management 
- Assist in appropriate utilization of acute palliative care services through active and timely response to referrals, facilitation of transfers from ICUs and other units, and early discharge planning.
- Assist in the development, implementation, and evaluation of program goals, objectives, policies, procedures, and standards of care which contribute to the achievement of department objectives.

Minimum required qualifications for these opportunities include: 
- Current RN and NP unrestricted licenses in CA
- Masters of Science in Nursing 
- Prior clinical experience in a Palliative Care related area 
- BLS certification
- DEA Licensure
