Our Care Direct business is a comprehensive chronic care management solution that helps health plans engage high utilization members through a suite of medical management clinical solutions, including: In Home Intensive Care, Post Acute Transition of Care, SNF Medical management Services and Advanced Illness programs. 

Position Summary:
The focus of the Matrix Chronic Care team (Care Direct) is to engage high utilizer complex, health plan members in their home and to stabilize their health by identifying and closing gaps in care. The CD Nurse Practitioner develops and initiates member-centered care and treatment plans, removes barriers to achieving goals, ensures effective care coordination in between the member’s visits to their PCP’s, focuses on member satisfaction and a reduction in overall health cost trend.  The CP Nurse Practitioner functions as an Independent Provider in the member’s home.


Care Direct Nurse Practitioner (CDNP) will focus on the comprehensive, holistic management of a caseload of the Adult/Geriatric health plan members who have progressive, multiple chronic illnesses and recent high healthcare utilization. 

Introduces Care Direct program goals and services to members. Evaluates, diagnoses, treats, formulates and revises as appropriate, the medical treatment plan and manages member’s care to meet program and member goals. Documents in the electronic health record timely and accurately

Engages members to understand their goals and formulates a member-centered care plan based on patient goals of care and member needs, then initiates appropriate interventions to achieve goals

Conducts regular member visits based on their risk level to perform physical examinations, identify and close gaps in care, order laboratory tests, x-rays, special imaging studies and other diagnostic procedures and orders consultations as appropriate.

Works collaboratively with the patient, other members of the interdisciplinary chronic care team and the member's PCP/Specialists as well as other care support agencies to ensure alignment on goals, interventions and progress on care plan and treatment goals.

Ensures care coordination between care settings, care stakeholders, the health plan and the member

Provides on-call support to ensure program goals and objectives are met

Educational Requirements:
Board Certified Nurse Practitioner with specialty in Adult, Family or Primary Care
Master's Degree required and satisfactory completion of NP licensure

Minimum of 3 years' experience as NP evaluating and managing the adult/geriatric patients with chronic medical conditions in the facility and or home settings
Current NP licensure in state of residence and state where caring for members if they are different
DEA License (where applicable)
Must have experience working with the Microsoft Office Suite (Outlook, Word, Excel)
Experience in working in fast paced, growth oriented environment
EHR experience
Home Health/ House Calls Experience Preferred
