Seeking Hospice/Palliative Care Nurse Practitioner to join team in northern NH.
Hospice care program includes a certified hospice nursing assessment, 24/7 clinical support, in-home provider visits for pain and symptom management, medical supplies, equipment and hospice medications, volunteer support and companionship, social and spiritual support, and bereavement and grief support.

- Work 16 hours/week at Berlin/Colebrook area and 16 hours in Lancaster
- Provide medical services necessary to meet the needs of palliative care and hospice patient through outpatient visits and in-home visits and
- Provide inpatient consults and be on rotation through Provider call schedule.
