Facility: A Patient-Centered Medical Home (PCMH) and Federally Qualified Health Center (FQHC) providing comprehensive primary health care delivered through innovative and alternative systems of health delivery. Opportunity: Direct Hire Internal Medicine Nurse Practitioner * All Outpatient * No Call * Adult Patient Populations * New Haven and East Haven (may cover two sites, but only 6 miles apart) * Monday - Friday Schedules * Case Load: 16-18 patients per day Compensation: This is a full time, permanent position.
